(Typically a bank, credit union, or public library will offer this service.)

*If you choose Option 1 or 2  you will be status corrected, but won’t be part of an Assembly.*

Information About Recording:


- One Blue Pen
- One Red Pen
- Red Fingerprint Ink Pad (for your thumbprint)


- Your birth certificate, ID or birth paperwork  - to show during your recording
- Identification - Photo ID or Passport
 - For the Universal Declaration - Two - Three  Witnesses (Three if you are not using a notary.)




Your Declaration is autographed in blue ink including your right thumbprint (seal)
in red, and a copyright symbol © - see example:Autograph%20%26%20Thumbprint Example Autograph & Thumbprint - covers copyright symbol and last letter of autograph.

If your last name is different than your born name,
add; (born last name)
Example: Mary Jane Smith (born Jones)©
